Y3S Donation Request Form

Thank you for thinking of Your 3rd Spot in your fundraising endeavor. We take pride in supporting the community that supports us.

In order to be considered for a donation from Your 3rd Spot, please fill out the informaiton below in its entirety. Y3S receives a high volume of requests and we have constantly changing availability, therefore we are not able to fulfill all requests. All requests are reviewed within 4 weeks and you will be notified only if you qualify based on Y3S requirements.

Donation and fundraising request forms that are submitted incomplete or do not qualify based on Y3S Requirements listed below will not be approved. Be assured that we receive all requests and, if your organization qualifies and is chosen, you will receive notification prior to your event. No follow up is needed.

Your residence or organization is within a 10 mile radius of Your 3rd Spot Your request is at least 8 weeks before the scheduled event date You are able to submit a request with 501(c)(3) and tax ID# on Charity letterhead
The Your 3rd Spot Community Sharing Program is not able to support the following:
- Religious organizations or programs
- Individuals seeking aid
- Agencies that receive United Way funding
- Groups lacking proof of 501(c)(3) status
- Political organizations or candidates
- Conferences; seminars; or reunions
- Research projects
- Travel for individuals or groups
- Capital campaigns
